The first time you log in your default user ID is your Social Security Number (123456789)
and the password is your date of birth (mmddyyyy).

Once on the "Plan Selection" screen:

1. Click the appropriate plan link
2. Click on the "Initiate Requests" tab
3. select "Investment Provider Elections" and complete the online form
